To ensure we are meeting your needs, we ask all Self-Exclusion participants to complete a survey to help us make improvements to the Program.
Your feedback is important to us. It will help us learn what we do right and what we can do better.
The survey should only take about five minutes to complete. The survey results will be kept confidential and anonymous.
