This form cannot be submitted until after your casino has been completed, and must be completed by a member of the organization with signing authority.

An organization may amend its use of gaming proceeds at any time by submitting the request on this form.  

This form must be submitted and approved for all donations in excess of $5,000.00 before gaming proceeds can be used for donations out of Alberta.

This form must be submitted and approved before gaming proceeds can be used for wages/salaries.

Retention of Gaming Funds must have prior approval from AGLC
